House of Wellness – General Rules

  1. Reservations are required by calling, emailing or reserving your time electronically into Jonas Software/Member Self-Service Portal “Empower M.E.”
  2. Daily temperature check is required.
  3. Masks worn at all times unless a medical waiver is presented.
  4. 6 Feet Social Distancing is required.
  5. Membership Application, Par-Q, Facility Orientation, Consent & Release Waiver, Youth Waiver, COVID-19 & Infectious Disease Waiver must be completed prior to utilizing the fitness center.
  6. Must scan in with House of Wellness Welcome Desk.
  7. There are disinfectant and hand sanitizer stations throughout the facility. It will be required that you disinfect your equipment thoroughly after use.
  8. No loitering will be allowed in or around the House of Wellness.  Please have your transportation planned after your reservation time is completed.
  9. No food is allowed with the exception of water bottles. Water fountains are not available.  
  10. No showers accessible in the locker rooms and no hand towels will be provided.
  11. No new membership sales to community members w/ the exception to HC Employees during phase II.
  12. No in person group exercise classes will be offered during phase II.
  13. Facility is not responsible for injuries, illness and/or lost, stolen or damaged property.
  14. Children under the age of 16, must be accompanied and supervised by a parent or guardian at all times.
  15. Members are responsible for all damage caused by negligent use of facilities and/or equipment.
  16. No foul, obscene, derogatory or offensive language at any time.
  17. No smoking, alcohol, under the influence drugs or alcohol allowed at any time.
  18. Parking in designated parking areas only.
  19. Requests made by staff for safety, security reasons and for compliance with these general rules must be complied with at all times.
  20. Staff will issue only 2 warnings for any violations of the general rules, violators will be asked to leave the facility.  Violators refusing to leave will be subject to charge of criminal trespassing and arrest/detention by the local authorities and loss of privileges with the use of House of Wellness S2845 White Eagle Rd. Baraboo, WI 53913.
  21. Report damages to the House of Wellness Security staff and restitution made before use of public areas will be allowed.
  22. If a positive COVID-19 test result from a member that has used the facility, a 12 hour closure will take place to deep clean and sanitize the facility.
  23. Management reserves the right to change hours of operation depending on facility needs.

    Mon-Thurs:  630am – 130pm
    Friday: CLOSED
    Saturday: CLOSED
    Sunday: CLOSED

    1 hour maximum/member/day.

    Bring your own towel & water bottle.

    No Group Exercise Classes at this time.

    No new membership sales at this time.
